Returning CTYFL football players and cheerleaders must provide following:

1. CTYFL Registration & Physician Consent Form
2. Registration Payment
3. Report Card From Prior Quarter (A Team Participants Only)

New football players or cheerleaders must provide the following:

1. CTYFL Registration & Physician Consent Form
2. A Copy of Player or Cheerleader's Birth Certificate
3. Registration Payment
4. Report Card From Prior Quarter (A Team Participants Only)
